Responsibilities

• Provide comprehensive outpatient primary care services to patients of all ages.
• Diagnose and manage acute and chronic medical conditions.
• Perform preventive care, annual physicals, wellness visits, and health screenings.
• Develop individualized treatment plans based on patient needs.
• Coordinate care with specialists and interdisciplinary healthcare teams.
• Order and interpret laboratory and diagnostic testing.
• Educate patients and families on preventive health and chronic disease management.
• Maintain accurate documentation in the EMR.

This role offers an excellent opportunity to provide comprehensive, outpatient primary care to patients of all ages, from pediatrics through geriatrics, within a collaborative care environment. Responsibilities

• Provide comprehensive outpatient primary care services to patients of all ages
• Diagnose and manage acute and chronic medical conditions
• Perform preventive care, annual physicals, wellness visits, and health screenings
• Develop individualized treatment plans based on patient needs
• Coordinate care with specialists and interdisciplinary healthcare teams
• Order and interpret laboratory and diagnostic testing
• Educate patients and families on preventive health, chronic disease management, and overall wellness
• Maintain accurate and timely documentation within the electronic medical record (EMR)

Shift & Schedule

• Monday-Friday schedule, 9:00 AM-6:00 PM
• No weekends
