CapitalCare Family Practice Great Oaks, part of Community Care Physicians, Albany, NY, is seeking a NP/PA to provide comprehensive family medicine care across all ages. The role offers a 4-day, 40-hour work week, collaboration with a physician-led multispecialty network, and strong administrative support.

Starting salary is $125,000+ with a sign-on bonus up to $7,500 in the first year. CME time and CME allowance are included to support ongoing professional development and patient-centered care.

Formación

• Current New York State NP or PA license.
• Graduate of an accredited NP or PA program.
• Family medicine or primary care experience preferred.

Responsabilidades

• Provide comprehensive primary care to patients across the lifespan.
• Perform physicals and wellness visits, acute visits and follow-up care.
• Evaluate, diagnose and manage acute and chronic conditions.
• Provide preventive health counseling and patient education.
• Coordinate care with specialists and other health team members.
• Utilize telemedicine when appropriate.
• Maintain accurate and timely electronic medical records.

About Community Care Physicians

Community Care Physicians is a physician-led, independent multispecialty medical group serving the Capital Region. CCP provides the clinical, technological and administrative resources that allow providers to focus on patient care while maintaining the collaborative feel of a community-based practice.

• License and active registration as a physician assistant or registered nurse/nurse practitioner in New York State.
  • Physician Assistant:
    • Bachelor's or equivalent degree in physician assistant studies and successful completion of physician assistant training program approved by the NYS Department of Education or accredited by the Accreditation Review Commission on Education for the Physician Assistant, or a program that is determined by the Department to be the substantial equivalent of a licensure qualifying or accredited program.
    • Attainment of passing grade on the Physician Assistant National Certifying Examination administered by the National Commission on Certification of Physician Assistants.
    • Current certification by National Commission on Certification of Physician Assistants.
    • At least one year of experience as a physician assistant in ambulatory health care.
    • Current CPR certification.
  • Nurse Practitioner:
    • Successful completion of and graduation from a nurse practitioner education program acceptable to NYSED.
    • Current certification as a nurse practitioner by the American Academy of Nurse Practitioners Certification Board (AANPCB) or the American Nurses Credentialing Center (ANCC).
    • Successful completion of pharmacology coursework acceptable to NYSED.
    • Three years of professional nursing experience in an ambulatory setting preferred.
    • At least one year of experience as a nurse practitioner in ambulatory health care.
    • Current CPR certification.
