The APRN/PA in Primary Care functions at a medical level of care to serve patients with primary care needs (acute, chronic, preventive) inclusive of adults and/or pediatric populations. The APRN/PA may function as a PCP maintaining a panel and/or via a care team model co‑managing patient panels with physician partner(s). The APRN/PA participates as part of an interdisciplinary team to provide safe, high quality, cost‑effective, and evidence‑based care. The APRN/PA demonstrates the highest levels of professionalism and upholds the missions, visions, and values of Advocate Health. The APRN/PA will provide care within their scope of practice as outlined by applicable state law, licensing, regulations, institutional policy, privileges, and practice agreements.
