Qualifications

• Active or eligible license in AZ as a Nurse Practitioner (NP) or Physician Assistant (PA)
• Current national board certification (ANCC, AANP, NCCPA)
• BLS certification
• Minimum of 3 years of experience as an Advanced Practice Provider in Family Medicine or Primary Care
• Proficiency with Electronic Health Records; Cerner is a plus
• Tech‑savvy with the ability to learn new technologies quickly

Responsibilities

• Deliver compassionate, expert primary care within the Family Medicine Department in Queen Creek, AZ
• Conduct thorough assessments and develop individualized treatment plans
• Collaborate with a multidisciplinary team to support patient and family well‑being
• Take patient history, perform exams, order tests, diagnose, and provide preventive care
• Assist in patient admissions/transfers and make referrals to specialists
• Participate in teaching, quality improvement, and research within scope of practice
• Engage in research or scholarly activity and work with a multidisciplinary team to develop care plans
