Zohorecruit is seeking a Family Medicine Physician to join our Post-Acute Care division serving the Jacksonville area. The role focuses on delivering high-quality care to skilled nursing facility patients across St. Augustine and Jacksonville, with rounding support and on-call coverage.

Responsibilities include managing 20-22 patients per day, using the Nursing Home EMR, and providing care across 3 buildings as needed. Competitive salary, PTO, benefits, and CME stipend are included.

We are seeking a compassionate and experienced Family Medicine Physician to join our dynamic Post-Acute Division. This full-time position offers a rewarding opportunity to provide high-quality care to our skilled nursing facility patients in the St. Augustine and Jacksonville areas.

Family Medicine Position Responsibilities:

Rounding:

• NP support to assist with patient care.
  
• Manage a patient load of 20-22 patients per day.
  
• Utilize Nursing Home EMR for efficient charting.
  
• Provide comprehensive care to skilled, short-term patients with 3 visits per week.
  
• Attend to 3-5 buildings as needed.
  

On-Call:

• Day call coverage from 8 AM to 6 PM for facilities rounded on that day.
