Job summary

Central City Community Health Center seeks physicians to practice across Family Practice, Internal Medicine, Pediatrics or Obstetrics/Gynecology at multiple sites. You will work as an extension of the Medical Director and ensure high-quality patient care.

You must hold an active California medical license and be able to perform comprehensive clinical duties, including prescribing and documentation. Bilingual Spanish/English is preferred to serve our diverse community.

Qualifications

• Graduation from an accredited Medical School and Residency.
• Active California Medical License required.
• Active DEA registration required.
• Bilingual in Spanish and English preferred.
• Board certification in Family Practice desirable.

Responsibilities

• Provide medical services across Family Practice, Internal Medicine, Pediatrics or OB/GYN at designated sites.
• Perform diagnostic interviews, clinical examinations, evaluations and charting of patient records.
• Answer after-hour calls on a rotational basis.
• Provide outpatient hospital services at clinics and partner facilities.
• Maintain confidentiality and comply with CCCHC policies and regulations.
• Complete reports/documentation including billing in a timely manner.

JOB SUMMARY: Responsible for engaging in the practice of medicine in the area of Family Practice, Internal Medicine, Pediatrics or Obstetrics/Gynecology at designated sites, functioning as an extension of the Medical Director and/or the Chief Medical Officer at a local level.

ACCOUNTABLE TO: Chief Medical Officer

JOB DUTIES: Responsible for performing the duties of a licensed physician such as the performance of diagnostic interviews, clinical examinations, evaluations, pharmaceutical prescribing, telephone consultations, preparation of clinical reports, and charting of individual patient medical records, including the following:

• Responsible for providing all services in a manner that meets or exceeds the community standard of practice and ethical requirements of the medical profession.
• Responsible for answering after-hour calls for all sites on a rotational basis.
• All related travel, record keeping, charting and other services related to the services described above (hereinafter referred to as “Medical Services”).
• Providing out-patient hospital services for all patients of the clinics at various facilities as assigned.
• Responsible for being credentialed with the hospitals CCCHC partners with for continuity of care compliance.
• Responsible for providing medical services to various facilities such as clinics and residential board and care facilities as assigned.
• Responsible for complying with all policies, standards, and regulations of CCCHC and all governmental or medical regulatory authorities having jurisdiction over employee’s and/or employer’s practice of medicine.
• Responsible for maintaining the confidentiality of patient information and records in accordance with applicable state and federal laws, as well as in compliance with CCCHC policy.
• Responsible for utilizing the medical forms and records provided by CCCHC, which shall remain the property of CCCHC
• Responsible for providing in-service education of staff.
• Responsible for providing other health services or procedures as requested from time to time by CCCHC.
• May be responsible for being the lead provider at designated sites, functioning as an extension of the Medical Director at a local level.
• Complete reports and appropriate documentation, including billing and medical records, in a timely manner.
• Consistently demonstrate and uphold CCCHC’s principle of providing quality health and human services to the medically underserved and low-income populations in a culturally sensitive manner.
• Punctuality and Attendance: This is an essential job duty for CCCHC’s employees given the impact on patients.
• All other duties as assigned.

Education and Experience Requirements

• Graduation from an accredited Medical School & Residency program for Physician required.
• Active California Medical License required.
• Active DEA required.
• Bilingual in Spanish and English preferred.
• Board certification in Family Practice other primary care discipline desirable.
