Job summary

Lifelineyouth is seeking a qualified professional to provide home-based and community-based services across adoption, guardianship support, and family preservation programs. You will conduct visits, deliver treatment, maintain documentation, and prepare required reports in line with service standards and organizational guidelines.

The role requires a bachelor's degree in a related field, 18+ months in adoption/guardianship or child welfare, and 21+ years of age, with CPR/First Aid certifications

Responsibilities

• Provide structured goal-oriented, time-limited services with clients per referral and treatment goals.
• Provide Adoption & Guardianship Support Services as needed and assigned.
• Provide care according to evidence-based practice standards.
• Offer 24-hour emergency/crisis consultation support to clients.
• Meet with clients regularly as determined by level of care and referral source.
• Provide transportation for clients to meet treatment goals.
• Engage in weekly staffing and address risk management for safety and treatment.
• Report significant information to referral source/supervisor about incidents.
• Engage client participation in the treatment planning process.
• Accompany and support clients for court appearances as requested.
• Coordinate and case-manage educational, vocational, recreational services as needed.
• Demonstrate respect and cultural sensitivity to all clients and referral sources.
• Maintain confidentiality of client information.
• Complete documentation of billable/non-billable contacts, monthly reports, court reports, and treatment plans.
• Demonstrate knowledge and skills to implement program objectives.
• Attend mandatory in-services/meetings.
• Attain/maintain CPR and First Aid certifications.

This position provides programmatic services across Home‑Based Services, Adoption and Guardianship Support Services, Family Preservation Services, and Family‑Centered Treatment. Responsibilities include conducting home visits and delivering treatment services, maintaining accurate documentation of all client interactions, and preparing required reports in alignment with service standards and organizational guidelines.

Requirements

Major Responsibilities / Activities

• Provide structured goal‑oriented, time limited services with clients as guided by their referral, case plan, and treatment goals.
• Provide Adoption & Guardianship Support Services as needed and as assigned.
• Provide care according to evidence based practice standards.
• Provide ongoing support for clients, including emergency and crisis consultation on a 24‑hour basis.
• Meet with clients on a regular, consistent basis. The frequency of visits is determined by level of care needed on a case‑by‑case basis and/or by the referral source.
• Provide transportation for clients, as needed or necessary, to meet treatment goals.
• Engage in weekly staffing and address risk management areas to ensure safety of clients and effective treatment.
• Report to referral source / supervisor significant information regarding any unusual incidents occurring within area of responsibility.
• Engage and assist the client’s participation in the treatment planning process.
• Accompany and support clients for court appearances, as requested by the referring agency.
• Coordinate and case manage appropriate educational, vocational, recreational, and other services, as needed.
• Demonstrate respect and cultural sensitivity to all clients and to all referral sources.
• Maintain strict confidentiality at all times to protect clients' privacy.
• Comply with required in‑service / continuing education hours.

• Completion of clear, accurate, and timely documentation of 1.) all billable and non‑billable contacts 2.) monthly reports, 3.) court reports, and 4.) treatment plans.

• Attain / maintain CPR and First Aid certifications.

Travel

Travel typically includes driving typically within assigned region and will be determined by the needs of the clients. An estimation of 200 – 500+ miles per week may be required.

Physical and Visual Acuity Requirements

The physical requirement of this position is defined as Medium Work: Lifting up to 50 pounds frequently.

The visual acuity requirements needed to perform this position include having close visual acuity to performing activities such as: preparing and analyzing data and figures; viewing a computer terminal; and reading.
