We deliver evidence-based, family-focused and youth-centered services that make a difference. That means that everything we do is backed by the most recent research and focused on the needs of your child and your whole family. The Care Transitions Team at Children's Hospital Colorado supports the coordination and wrap around care for children with behavioral health needs as they go between hospital and community services. The goal of this team is to facilitate the movement along the care continuum and eliminate barriers for families in accessing needed mental health services. This interdisciplinary team includes behavioral health placement specialists, behavioral health care coordinators, education specialists, family navigators, and family and peer support partners who join together to support children and families and help them to successfully navigate the mental health system of care.
The ideal candidate will have the ability to set clear boundaries, is proficient at reading and writing, has active listening skills, is open and receptive to learning and receiving feedback, and has an understanding of confidentiality and patient privacy. In addition, experience working on a multidisciplinary team is preferred but not required.
Pay is dependent on applicant's relevant experience.
Hourly Range: $22.21 to $33.31
