Job summary

Mental Health Resource Center, Inc. is seeking a Full Time Licensed Practical Nurse (LPN) to conduct psychiatric assessments, coordinate referrals and ensure safe medication administration for FACT participants. Most services are community-based, with some office time, and mileage reimbursement available.

The FACT team operates with rotating weekends and on-call responsibilities. Position requires Florida LPN licensure, 6 months of relevant nursing experience, and proficiency with the MHRC RBHS

Qualifications

• Graduation from an accredited Practical Nursing program.
• Valid Florida LPN license in good standing.
• Six months of medical/surgical and/or psychiatric nursing experience.
• Proficiency with RBHS/MHRC EHR and patient information systems within three months.
• Proficiency in Microsoft Office, Outlook and Internet usage.
• Must have a valid Florida driver’s license and clean driving record.
• Ability to work in homes, offices, hospitals and secure units.
• Strong communication and interpersonal skills.

Responsibilities

• Provide treatment, rehabilitation and support services per the FACT model.
• Maintain accurate, confidential clinical records.
• Conduct mental health status assessments.

• Educate clients on mental health symptoms, medications and recovery.

Mental Health Resource Center is a not‑for‑profit Florida corporation that provides a wide range of mental health and behavioral health care services to the community. Services include 24‑hour emergency services, inpatient psychiatric services for children, adolescents and adults, as well as outpatient services such as medication management, case management and counseling.

Job Summary

The FACT Licensed Practical Nurse (LPN) conducts psychiatric assessments, assesses physical health needs, coordinates referrals to community health care providers and ensures proper medication self‑administration in conjunction with the Psychiatrist or Psychiatric Advanced Practice Registered Nurse (APRN). The LPN serves as primary case manager for an assigned group of individuals and promotes recovery, empowerment, self‑determination and decision‑making for each FACT participant.

The FACT (Forensic Assertive Community Treatment) Program is a multidisciplinary clinical team that provides comprehensive, community‑based services to adults with severe and persistent mental illness. The program is designed to reduce hospitalization, increase community tenure and enhance independence, autonomy and quality of life.

FACT team members provide a wide range of services, including advocacy, crisis intervention, assessments, treatment planning, case management, medication support and coordination of care. Additional services include community referrals, symptom education, vocational and educational support, social skills development and assistance for families and caregivers.

Most services are provided in the community, with approximately 15% of time spent in the office. Company vehicles are available; however, team members also use their personal vehicles as needed and are eligible for mileage reimbursement.

The FACT team operates during and outside of standard business hours, including rotating weekend, holiday and on‑call responsibilities.

Responsibilities

• Provide treatment, rehabilitation and support services in accordance with the FACT model (based on the PACT model).
• Maintain accurate, complete and confidential clinical records.
• Conduct mental health status assessments.
• Coordinate referrals to community providers and support services.
• Provide education on mental health symptoms, medications and recovery.
• Ensure proper medication self‑administration in conjunction with the ARNP or Psychiatrist.
• Serve as primary case manager for an assigned caseload, promoting recovery, empowerment and self‑determination.

• Reduces time spent on documentation, allowing more focus on client interaction and care.
• Transforms brief notes or session conversations into complete, clinically appropriate documentation.
• Supports accurate, compliant documentation while allowing staff to review and finalize notes.
• Provides helpful insights from sessions to support care planning and continuity.
• Accessible on desktop or mobile for flexibility across different work settings.

Qualifications

• Graduation from an accredited school of Practical Nursing required.
• Valid Florida Licensed Practical Nurse (LPN) license in good standing required.
• Six months of medical/surgical and/or psychiatric nursing experience required.
• Proficiency in the RBHS/MHRC Electronic Health Records (EHR) and Patient Information System demonstrated within three months of employment.
• Proficiency in Microsoft Office, Outlook and the use of the Internet.
• Must meet MHRC/RBHS driver requirements, including a valid Florida driver’s license, required insurance coverage and acceptable driving record.
• Ability to work in a variety of settings, including homes, offices, hospitals, nursing homes and secure units.
• Ability to make sound, independent decisions, including responding to emergencies and high‑risk situations.
• Strong communication and interpersonal skills; ability to work effectively with individuals, families, providers and team members.

Position Details

This position is a Full Time position (rotating weekends, on‑call and holidays).
