Job summary

Community Health & Dental Care, Inc. seeks an Express Care Provider to deliver rapid, patient-centered care for acute illnesses in a fast-paced ambulatory setting.

You will evaluate, diagnose, and treat patients across the lifespan, performing procedures within scope and collaborating with physicians. The role offers flexible scheduling with 10- or 12-hour shifts, weekday/evening/weekend rotations, and opportunities to work with the Medical Mobile Unit as needed.

Qualifications

  • 1-2 years’ experience in both Adult and Pediatric Settings.
  • Current RN licensure in Pennsylvania.
  • Current Nurse Practitioner Certification in Pennsylvania.
  • Certification as a Nurse Practitioner by a nationally accredited nursing organization.
  • DEA number preferred.
  • BCLS required.
  • ACLS preferred.

Responsibilities

  • Evaluate, diagnose, and treat patients presenting with acute, episodic, and non-life-threatening illnesses and injuries.
  • Manage a broad range of conditions, including URIs, influenza, UTIs, rashes, sprains, strains, lacerations, minor fractures, and other urgent care conditions.
  • Order, perform, and interpret diagnostic testing including laboratory studies, point-of-care testing, and EKGs.

Job description

Position Summary:

The Express Care Provider delivers high-quality, patient-centered care for individuals with acute illnesses and minor injuries in a fast-paced ambulatory setting. The provider evaluates, diagnoses, and treats patients across the lifespan, performs procedures within scope of practice, and collaborates with physicians and interdisciplinary team members to ensure efficient, evidence-based care. This role offers flexible scheduling opportunities, including 10-hour and 12-hour shifts, with weekday, evening, and/or weekend coverage rotations as needed.

General Purpose:

Provides professional medical services to complement the delivery of primary health care services; Works closely with the Chief Medical Officer and Chief Clinical Officer.

Role Qualifications:
  • 1-2 years’ experience in both Adult and Pediatric Settings

  • Current Nurse Practitioner Certification in Pennsylvania

Attendance required at assigned CHDC site(s) as needed or required.

Position Responsibilities:
Clinical:
  • Evaluate, diagnose, and treat patients presenting with acute, episodic, and non-life-threatening illnesses and injuries.
  • Manage a broad range of conditions, including upper respiratory infections, influenza, urinary tract infections, rashes, sprains, strains, lacerations, minor fractures, and other common urgent care conditions.
  • Order, perform, and interpret diagnostic testing including laboratory studies, point-of-care testing, and EKGs, as appropriate.

Perform procedures within scope of practice, including:

Laceration repair, abscess incision and drainage, splinting and immobilization, foreign body removal, wound care, and other minor based office procedures.

  • Provide patient education regarding diagnosis, treatment plans, medications, follow-up care, and preventive health measures.
  • Recognize medical emergencies and facilitate transfer to a higher level of care when clinically indicated.
  • Provides medical evaluation, treatment, and consultation services to patients with appointments on the Medical Mobile Unit.
  • Responds to emergencies and answers medical questions.
  • Obtains and documents appropriate chief complaint, past medical, family, and social history, review of systems, examination, medications and allergies, assessment, and plan for each patient.
  • Provides medical orders for prescription refills, administration of medications , ancillary testing, and/or referrals.
  • Reviews ancillary test results and coordinates notification to the patient.
  • Provides patient education as needed.
Essential Skills & Experience:
  • Previous family practice or urgent care experience preferred.
  • CPR, and all applicable certifications required (must be obtained within 90 days of employment).
  • Understands ethics of confidentiality.
  • Previous computer experience or training preferred.
  • Successful candidates must submit to TB testing and any vaccinations recommended by OSHA Guidelines.
