Job summary

St. James Rehabilitation and Healthcare Center is seeking an experienced Physical Therapist (PT) to assess referred patients and deliver direct rehab services. You will develop and oversee treatment plans, document progress, and educate patients and families while collaborating with multidisciplinary teams.

The role requires PT licensure, graduation from an accredited PT program, and experience in long-term/geriatric care. Full-time, on-site with competitive compensation and benefits expected.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Oversees and supervises Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care preferred.

Responsibilities

• Assess patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals per patient capacity.
• Document patient condition and educate patients and families in PT methods.
• Collaborate with all disciplines to provide best patient-centered care.
