Responsibilities

• Perform assigned resident care duties meeting physical, psychosocial, and spiritual needs of residents.
• Complete initial and ongoing assessments by gathering data promptly and considering age factors.
• Differentiate between normal and abnormal clinical findings and intervene per physician orders.
• Collaborate with resident/family and interdisciplinary team to develop an individualized care plan.
• Administer medications and provide IV care if certified, per policy.
• Document and report changes to RN, attending physician, family and nursing leadership.
