Job summary

Brighton/Suncrest Hospice is seeking a Weekend Registered Nurse to provide compassionate care for a caseload of hospice patients across private homes, assisted living facilities, and hospitals. The role requires completing admissions and visiting patients as needed, while supporting families and the care team.

Shift is Saturday and Sunday, 8a-8p. You must hold an active RN license in good standing, with about one year of experience, and be able to travel within the service area with reliable

Details

This position will work Saturday and Sunday 8a-8p. The Weekend Registered Nurse (RN) will be responsible for providing care and services for a group of hospice patients, and visiting patients where they reside, which could include private homes, assisted living facilities, and hospitals.

As an RN, you will complete hospice admissions and patient visits as needed. Applicants must have great clinical and communication skills. The Weekend RN is responsible for answering calls from patients, families and customers. The RN will make nursing visits for symptom management, deaths, and admissions as needed. This RN position must also be passionate about hospice care and be a great advocate for both hospice services and Suncrest Hospice.

Qualifications

• Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice with one year of experience under license
• Admissions and On-call experience preferred
• Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Work closely with patients on a 1-1 level but get strong support from a patient care team.
• Can work independently and on your own on a day-to-day basis with activities that include: phone, email, and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.
• Time management skills to manage visits, documentation, and meetings (i.e.: schedule visits, attend required meetings, and respond to urgent needs by prioritizing and triaging multiple concerns)
• Ability to learn an electronic medical records system.
• Must be willing to drive with reliable transportation, a valid driver's license, and auto insurance, to be able to travel across the service area
