Job summary

IU Health in Indiana seeks a pharmacist to join our hospital-based team, delivering comprehensive medication-related care and collaborating with physicians and nurses to improve patient outcomes.

The role requires a PharmD or BS in Pharmacy, possible PGY1/PGY2 residency, licensure eligibility in Indiana, and may involve travel and weekend rotations.

Qualifications

  • PharmD or BS in Pharmacy required or prior to 2006.
  • ASHP-accredited PGY1 or PGY2 residency may be required.
  • Requires 0-3 years of relevant experience.
  • Requires current Indiana Pharmacist license or eligibility for Indiana licensure.

Responsibilities

  • Provide value-based medication care to improve patient quality of life.
  • Collaborate with physicians, nurses and other staff to optimize medication therapy outcomes.
  • Ensure safety and effectiveness of the medication use process.
  • Process and dispense medications per order or prescription.
  • Monitor medication therapy and comply with regulatory requirements.
  • Maintain security of medications and confidentiality of patient information.
  • Provide medication information support to medical and nursing staff.
  • Identify challenges in medication use and intervene when necessary.

Education

PharmD or BS in Pharmacy
ASHP PGY1/PGY2 residency (may be required)
Indiana pharmacist licensure eligibility
Board certification or specialized training (may be required)

Description

Provides comprehensive, value-based medication-related care to achieve definite outcomes that improve a patient's quality of life. Acts in collaboration with other healthcare professionals in the shared responsibility for patient medication therapy outcomes. Key job duties may consist of ensuring safety and effectiveness of the medication use process, processing and dispensing medications per order or prescription, medication therapy monitoring, regulatory compliance, maintaining oversight over pharmacist extenders, maintaining the security of medications, and protecting the confidentiality of patient information. Provides medication information support for medical and nursing staff, ensuring continuity of care, and identifies potential challenges and intervenes, if necessary, to medication-related issues.

  • Requires Doctor of Pharmacy (PharmD) or Bachelor's of Science in Pharmacy prior to 2006.
  • Successful completion of an ASHP accredited PGY1 or PGY2 residency may be required.
  • Requires 0-3 years of relevant experience.
  • Requires current Indiana Pharmacist license or eligibility for Indiana licensure as a Pharmacist.
  • Board certification or other specialized training may be required.
  • Basic Life Support certification through the American Heart Association (AHA) may be required.
  • May require travel to IU Health facilities.
  • May require weekend rotations.
