Northern Path Recovery Center, Fort Wayne, Indiana, is seeking a Licensed Counselor for the evenings to provide treatment, counseling, and family support to clients recovering from addictions. The role emphasizes assessments, treatment planning, and group or individual sessions with a focus on evidence-based modalities.

The position requires a Master’s degree in a related field, licensure, and at least 1 year of post-Masters experience in addictions or counseling.

Qualifikationen

• Master's degree in social services, addictions, mental health or closely related health services field.
• 1+ years of post-Masters degree professional experience in the addictions, therapy or counseling field.
• Must have current and valid licensure or certifications as a social worker, professional counselor, therapist or addiction counselor per state agency.

Aufgaben

• Conduct client assessments focusing on history, current presentation, problem areas and ongoing clinical need for treatment and risk assessment.
• Conduct ongoing assessments of chemical dependence services required by the treatment program.
• Prepare and carry out initial treatment planning and treatment plan progress reviews as assigned and document as required.
• Lead group counseling, individual or family sessions and implement evidence-supported treatment modalities.
• Provide client and family education related to substance abuse/psychiatric issues.
• Ensure all documentation regarding client care, treatment and incidents is completed timely and clearly.
• Provide crisis intervention and counseling to clients as needed.
• Plan and complete aftercare documentation for assigned clients.
• Act as liaison with insurance carriers and referral sources.
• Provide case management to ensure individualized quality care.
• Conduct educational workshops within the hospital and community.
• Collaborate with self-help groups and EAP professionals.
• Evaluate client needs to determine appropriate level of care and referrals.
