Trilogy Health Services seeks an Evening Supervisor (LPN, RN) to support the DON and ADON in guiding the Nursing Department’s evening operations at Bridgepointe Health Campus in Vincennes, IN.

The role requires active supervision of resident care, adherence to care plans, and collaboration with care teams to maintain high-quality, person-centered care.

Responsibilities

• Plan, develop, organize, implement, evaluate, and direct nursing services per guidelines.

• Assist with resident documentation in MatrixCare to ensure records are complete.
• Maintain first impressions and support campus readiness for surveys.
• Ensure campus is tour ready, manage tour resources and stations.
• Provide patient care as necessary.

Position Overview

The Evening Supervisor (LPN, RN) is primarily responsible for assisting the Director of Nursing (DON) and Assistant Director of Nursing (ADON) in overseeing the evening functions of the Nursing Department(s) to ensure that the highest degree of quality care is always maintained.

Key Responsibilities

• Assist the ADON & DON to plan, develop, organize, implement, evaluate, and direct the nursing service department, as well as its programs and activities, in accordance with current rules, regulations, and guidelines that govern the Health Campus.
• Supervise, oversee and assist with all resident care according to assessed resident status and established plan of care.
• Provide direct nursing care as necessary per each resident’s assessment, physician orders and plan of care and assess residents for expected results of treatments and medications.
• Assist in the scanning and attaching of resident documentation in MatrixCare to ensure Medical Records are complete and accurate.
• Maintain first impressions to welcome potential customers to experience our services and participate in survey and mock-survey campus preparedness.
• Ensure campus is tour ready including back up team resources, tour path, model room, first impressions, and tour stations. Execute tours as necessary.
• Provide patient care as necessary.

Work Requirements

• Evening shift hours, start time at the discretion of the DON
• 8-hour shifts
• Monday - Friday with on call rotation commitment

Qualifications

• Must have and maintain a current, valid state LPN or RN license
• Current, valid CPR certification required

Compensation will be determined based on the relevant license or certification held, as well as the candidate’s years of experience.
