Responsibilities

• Screens charts for historical and clinical information, conducts psychosocial evaluations and formulates treatment plans.
• Provides individual and group counseling and concrete social services in health care or community settings.
• Collaborates with other disciplines to develop multidisciplinary treatment plans and discharge/aftercare planning.
• Provides outreach and referrals to community agencies; supports high‑risk patients.
• Prepares reports and maintains records; ensures compliance with policies and standards.
• Mentors staff and may supervise students or volunteers; may provide services under supervision of a licensed clinician.
