Job summary

LiveWell Homecare Agency is seeking an Inpatient Licensed Practical Nurse (LPN) to deliver direct patient care in a clinical setting, assess needs, and support discharge planning.

This role requires graduate from Specialty Training Program in Nursing, strong interpersonal and communication skills, and ability to collaborate with the healthcare team. Evening shifts with every other weekend and holiday requirements are included to support patient care continuity.

Responsibilities

• Contribute data to patient assessments and care planning.
• Address physical, psychological, social, cultural, and spiritual needs of patients and families.
• Provide treatments, medications, and diagnostic studies per policies.
• Evaluate intervention effectiveness and adjust plans accordingly.
• Collaborate with interdisciplinary team members, including physicians and ancillary staff.
• Mentor and precept new staff and students, fostering a professional environment.
• Coordinate discharge planning and patient education.
