Aliya Healthcare Consulting is seeking a dedicated Licensed Practical Nurse (LPN) to join our compassionate care team at our skilled nursing facility. You will provide total nursing care, supervise team members, and ensure compliance with facility standards while supporting resident well-being and professional development.

Qualifications

• Graduation from an accredited nursing education program.
• Active LPN license in the state of Illinois.
• No prior abuse convictions.
• Strong communication and teamwork skills.
• Ability to follow policies, maintain safety, and provide empathetic care.
• Ability to work with diverse resident populations.

Responsibilities

• Provide total nursing care to residents on the assigned unit during the shift.
• Develop and implement individualized care plans with the healthcare team.
• Administer medications and treatments as prescribed and assess effectiveness.
• Maintain an environment that supports residents’ well-being.
• Educate residents and families on care plans and procedures.
• Monitor team member performance and assist with development, evaluation, and corrective action if needed.
• Delegate and ensure completion of assigned nursing tasks to team members.
• Stay current with facility policies, procedures, and ongoing nursing education.
• Recognize and act on significant changes in resident conditions.
• Document care, responses, and relevant information per facility guidelines.
• Act as a charge nurse: answer questions, resolve issues, and support resident care decisions
• Follow all facility safety policies and procedures
