Job summary

Heritage Manor Care is a skilled nursing facility seeking a Licensed Vocational Nurse (LVN) to join our clinical team. The role focuses on delivering direct nursing care to residents and supervising nursing assistants to ensure high-quality care.

This full-time position runs 3pm-11pm. The LVN will admit, discharge, and transfer residents as needed, prepare and administer medications, and collaborate with physicians to support care plans.

Qualifications

• Provide direct nursing care to residents.
• Assist with care plans and care coordination.
• Supervise nursing assistants and day-to-day activities.
• Maintain charting and documentation as required.
• Administer medications as prescribed and monitor effects.

Responsibilities

• Complete and file required charts and treatment plans/physician orders, etc.
• Admit, transfer, and discharge residents as required.
• Prepare and administer medications as ordered by the physician
• Make rounds with physicians as requested/necessary.
• Orient and welcome new residents and families.
• Answer call lights and provide nursing assistance.
• Participate in and make recommendations for care plans.
