Job summary

Grey Stone Health & Rehab is seeking a Licensed Practical Nurse (LPN) for a weekend option on the evening shift at our Fort Wayne, Indiana facility. You will provide compassionate nursing care, assess residents, and document changes while building relationships with residents and staff.

You will collaborate with the care team to ensure coordinated, high-quality geriatric care, upholding CARE values and supporting a positive, respectful work environment on-site.

Qualifications

• Current and valid Licensed Practical Nurse license in Indiana.
• Ability to conduct thorough assessments and accurately document changes in resident condition.
• Strong passion for geriatric nursing and commitment to senior care excellence.
• Excellent communication and interpersonal skills.
