101 Clifton Springs Hospital & Clinic is seeking an Licensed Practical Nurse Staff I to join the Long Term Care Dementia Unit. You will provide professional nursing care to residents, supervise CNAs, and ensure accurate charting and documentation in a hospital-connected long-term care setting.

The role emphasizes medication administration, care planning, and collaboration with physicians; candidates should have NY LPN licensure, BLS, and prior long-term care experience.

Qualifications

• Graduate from an accredited School of Practical Nursing.
• 1 year experience preferred.
• Current licensure in the State of New York.
• BLS certification required.

Responsibilities

• Administer medications for assigned team with accurate documentation.
• Perform treatments and document progress per physician orders.
• Ensure complete resident care documentation in the EMR.
• Direct CNAs to deliver quality individualized personal care.
• Report and document changes in residents' status to the charge nurse or physicians.
• Assist in ADL care and support the CNA team as needed.
