Job summary

Ascension Living St. Francis Place in Milwaukee, WI is seeking an LPN to provide basic bedside care to long term care residents under the supervision of an RN or physician, on an evening shift as a full-time role.

You will assist with assessments, administer medications, monitor for adverse reactions, perform routine labs, and educate residents and families about treatment plans, self-care and discharge planning. A valid LPN license and BLS are required, with a high school diploma or equivalency.

• Collect data for RN assessment
• Administer medications and monitor for adverse reactions
• Perform routine laboratory tests and simple therapies
• Educate patients and families about treatment plans and post-discharge care

Responsibilities

• Provide basic bedside care to long term care patients under the direction of an RN or physician
• Contribute to the assessment of patients by collecting data for analysis by an RN
• Administer medications and observe patients for adverse reactions
• Perform routine laboratory tests and simple therapeutic services
• Educate patients/families about disease treatment plan including self-care post discharge
