Qualifications

Responsibilities

• Pass medications to elders within med aide scope.
• Report changes in elder condition to clinical team.
• Assist with personal care: bathing, grooming, dental care.
• Attend care plan meetings and discuss elder needs.
• Report accidents or incidents promptly.
• Assist with restorative/rehabilitative procedures.
• Observe infection control policies.
• Measure and record vital signs and measurements.
• Plan life enrichment activities for each elder.
• Document elder care in the EHR at time of service.
• Maintain elder confidentiality and privacy.
• Keep the household environment clean and safe.
• Participate in orientation and ongoing training.
