Qualifications

• Collaborates across disciplines to coordinate patient care including disposition and referrals.
• Effectively delegates care to ancillary personnel; may precept students and new hires.
• Plans, implements, and evaluates care based on patient assessment to optimize outcomes and resources.
• Monitors, documents, and communicates patient condition; provides patient and family education.
