Job summary

Qualifications

Responsibilities
- Conduct comprehensive assessments for patients across physiological and psychosocial domains
- Develop and implement individualized care plans
- Administer medications and treatments per orders and protocols
- Educate patients and families on conditions, treatments and self-care
- Coordinate with medical staff to ensure integrated patient care
- Maintain accurate documentation in patient records
