Job summary

Longmont United Hospital seeks an RN Clinical Coordinator to support department leadership by coordinating clinical and department activities, serving as a clinical resource and role model for staff. • Assess patient needs through observation, interviews, medical records and data.
• Develop and implement an individualized plan of care with interdisciplinary collaboration.
• Provide leadership and coordinate daily unit operations.
• Ensure safe patient care with appropriate staff mix and shift coverage.

Responsibilities

• Assess patients' physical, psychosocial, cultural and spiritual needs and intervene for optimal outcomes.
• Develop and implement patient care plans and observe outcomes with nursing standards.
• Lead and coordinate unit activities during assigned shift.
• Direct and prioritize shift activities to meet patient care and staff needs.
• Assure appropriate admissions and staffing for safe care.
• Act as liaison to the multidisciplinary patient care team.
