Job summary

• Collects patient data, analyzes assessment data, and develops care plans.
• Collaborates with the healthcare team to provide safe, healing care.
• Participates in discharge planning to ensure continuity of care.
• Delegates and coordinates duties of the healthcare team.
• Performs other job-related duties as assigned.
