Qualifications

Responsibilities

• Assess patients on admission and monitor condition on an ongoing basis.
• Plan and coordinate care based on assessments and patient needs.
• Educate patients and families to support learning needs and discharge planning.
• Collaborate with the multidisciplinary team to achieve desired outcomes.
• Initiate, verify, and adjust physician orders as needed and administer medications per policy.
• Advocate for patients while maintaining confidentiality and ethical care.
