Job summary

Martin Luther King, Jr. Community Hospital is seeking an RN Care Manager for per diem day shifts in the Emergency Room.

The candidate will support the physician and interdisciplinary team to optimize patient outcomes, ensure timely pre-certifications, and coordinate care transitions and discharge planning across the continuum. The role requires a BSN, California RN license, BLS, and experience in case management.

Qualifications

• Bachelor of Science degree in nursing preferred.
• CA Nursing license required.
• Bilingual Spanish preferred.
• BLS certification required.
• Case management certification preferred.

Responsibilities

• Coordinate discharge planning and care progression across the interdisciplinary team.
• Facilitate precertification and payer authorization.
• Communicate with physicians and payors to ensure timely care and reimbursement.
• Assess patient needs and barriers to discharge and coordinate transitions.
• Support utilization review and cost-effective care planning.
