Job summary

Yale New Haven Health is seeking an entry-level clinical pharmacist to join our hospital-based Pharmacy Department in Connecticut. The role supports CHAMP behaviors, diabetes and medication safety initiatives, and participates in education for staff and students while ensuring high-quality patient care. The ideal candidate will have a PharmD or B.S.

Qualifications

• Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree.
• Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience is desirable, but not required.
• Current Connecticut Pharmacist License, or eligible for licensure.

Responsibilities

• Demonstrate CHAMP behaviors and promote customer service quality.
• Conduct timely, accurate medication order review considering patient labs and vital signs.
• Demonstrate competency in pharmacy automation and technology for medication preparation and distribution.
• Adhere to departmental policies, procedures, protocols, and guidelines for medication safety and quality assurance.
• Provide education and training to pharmacy staff, students and residents, and health care team members.
• Contribute to departmental cost-reduction and revenue generation.
