Job summary

Yale NewHaven Health is seeking an entry level clinical pharmacist to join our Pharmacy Department. The role supports our CHAMP safety culture by practicing basic clinical pharmacist skills and promoting patient-centered care.

Responsibilities include medication order review, medication preparation, and distribution with attention to laboratory values, vital signs, and formulary management. A Connecticut pharmacist license or eligibility is required or on completion of residency desirable but

Qualifications

• Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree.
• ASHP-accredited PGY1 residency desirable but not required; hospital pharmacy experience preferred.
• Current Connecticut Pharmacist License or eligible for licensure.
• Demonstrates basic pharmacology and therapeutics knowledge; strong communication and teamwork.

Responsibilities

• Provide customer service quality and CHAMP behaviors.
• Conduct timely medication order reviews considering patient characteristics, labs, and vitals.
• Demonstrate competency in pharmacy automation and technology.
• Adhere to departmental policies, procedures, and guidelines; ensure medication safety and regulatory compliance.
• Educate and train pharmacy staff, students, and health care team members.
• Contribute to departmental cost-reduction and revenue generation.
