Job summary

Qualifications

• Bachelor’s degree in social services, addiction, mental health or closely related health services field.
• Professional experience not required; internship/training preferred.
• CPR certification and de-escalation training required (training available upon hire).

Responsibilities

• Conducts client assessments to identify history, presentation, and treatment needs.
• Performs ongoing assessments of services required by the program.
• Participates in treatment planning and reviews; documents progress.
• Conducts group, individual, and family sessions using evidence-based approaches.
• Provides client and family education on substance use and psychiatric issues.
• Ensures timely, clear documentation of care and incidents.
• Provides crisis intervention and seeks consultation as needed.
• Plans and documents aftercare for assigned clients.
• Acts as liaison with insurance carriers as requested.
• Provides case management for individualized care.
• Conducts educational workshops in hospital and community settings.
• Liaises with self-help groups and EAP professionals.
