Job summary

SUNY Downstate Health Sciences University in New York seeks a Family Medicine Nurse Practitioner to provide primary care to a panel of patients and walk-ins, under supervision of physicians.

Responsibilities include documenting care in patient records, reviewing labs and imaging daily for urgent follow-up, coordinating referrals, and supporting education of medical and allied health students. Community health events participation encouraged.

Qualifications

• New York State RN and NP licensure required.
• Master of Science in Nursing required.
• BCLS certification required.

Responsibilities

• Provide primary care to individuals and families, including own panel of patients and walk-in patients as needed.
• Document all care provided in the patients' medical records.
• Develop and manage patient care panel with participation in managed care programs and other direct patient care programs as applicable.
• Review daily laboratory and radiology reports to identify abnormal results for urgent follow-up.
• Ensure that patients are contracted to arrange additional care and that their primary providers are notified.
• Participate in education activities for medical students and allied health professional students.
• Participate in hospital and departmental health fairs and speak to community groups when requested.
