WelbeHealth PACE is seeking an Enrollment Specialist to guide eligibility and enrollment for seniors, coordinating with the IDT during intake and supporting the Outreach & Enrollment (O&E) team as needed. The role focuses on successful intake, enrollment of participants, and collaboration with families in the Newark area.
The position requires conducting home visits, managing enrollment steps, building relationships with prospective participants, and ensuring timely CRM documentation.

The Enrollment Specialist focuses on the successful intake and enrollment of our participants and coordination with the Interdisciplinary Team (IDT) as required to facilitate enrollment. The Enrollment Specialist is a member Outreach & Enrollment (O&E) and provides back-up and support to other areas of the team as needed.
