Optima Medical in Arizona is seeking a Care Management Enrollment Coordinator to join our team. You will coordinate the enrollment process for new and existing patients, ensuring a smooth transition into our care management program.

Responsibilities include educating patients about enrollment benefits, collecting information, maintaining records, and collaborating with the care management team for seamless transitions. Healthcare experience is preferred.

Chronic Care Management (CCM)

CCM services are designed to improve the quality of care, enhance patient outcomes, and reduce healthcare costs by addressing the complex needs of individuals living with chronic illnesses.
