Job summary

Kern River Transitional Care is seeking a Part-Time Activities Assistant to support the Activity Director in creating an engaging activities program for residents. You will help plan and conduct activities, assist with communication among staff and families, and help arrange transportation when needed.

Qualifications include a high school diploma or equivalent and preferably a year of long-term care experience.

Qualifications

• High school diploma or equivalent.
• Preferable one-year experience in a long term care facility.
• Ability to read technical procedures.
• Ability to read and comprehend policy and procedure manuals.
• Ability to effectively present information to managers and employees.
• Ability to apply fractions, percentages, ratios and proportions to practical situations.
• Ability to solve practical problems.
• Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Responsibilities

• Plan and conduct individual and group activities.
• Communicate with staff, residents, families and visitors.
• Develop monthly activity calendar and track attendance.
• Support plans to address quality deficiencies as needed.
• Assist with discharge planning and activity care plans.
• Arrange transportation for outings and appointments.
• Create a diverse activity schedule for all residents.
• Encourage self-initiated hobbies and reading.
• Maintain activity area clean, safe and organized.
• Complete quarterly progress notes as assigned.
