Job summary

Hartford HealthCare in Norwich, CT is seeking an Endocrinology Advanced Practice Provider (APRN or PA-C) to join the Endocrinology, Diabetes & Metabolism Team. You will provide follow-up care for complex endocrinology disorders with a strong emphasis on diabetes, working with a high-functioning group of providers.

The position offers APP mentorship, tailored EPIC templates, and dedicated administrative time.

Qualifications

• Eligibility to be licensed in Connecticut as a Physician Assistant (PA) or Advanced Practice Registered Nurse (APRN).
• Eligibility to be certified as a Physician Assistant (PA) or National Board Certification as a Family Nurse Practitioner (FNP), Adult NP (ANP) or Adult Gerontology Primary Care NP (AGPCNP).
• Endocrinology/diabetes experience is preferred.

Responsibilities

• Provide follow up with patients with complex endocrinology disorders, with a large focus on diabetes.
• Collaborate with a high functioning team of providers to deliver care.
• Participate in APP mentorship and onboarding; utilize EPIC templates with dedicated time.
• Support career advancement opportunities within the hospital system.
