Hackensack Meridian Health is seeking a Clinical Coordinator to support subspecialty endocrinology care. The role collaborates with the Clinical Program Manager and team to coordinate care for complex patients and families.

The nurse will provide professional nursing care, educate patients, and act as a liaison among families and subspecialists. The position supports five endocrinologists and involves care coordination, education, and documentation in the EMR.

Qualifications

• Bachelor's of Science in Nursing.
• Minimum of 3-5 years acute/primary care nursing experience.
• Proficient computer skills with MS Office and/or Google Suite.
• Excellent written and verbal communication skills.

Responsibilities

• Maintains a communication system ensuring timely information flow to providers and the multidisciplinary team.
• Interacts with patients and families during visits and by phone to assess needs and support care.
• Documents interventions, plans of care, and findings in the EMR.
• Collaborates with the team in assessment, planning, implementation and evaluation of care.
• Facilitates diagnostic testing, prescription refills, and other instructions requiring provider sign-off.
• Serves as a clinical resource and educator to staff and patients.
• Contributes to development and implementation of evidence-based guidelines.
• Supports continuous process improvements to improve outcomes and satisfaction.
