Job summary

Dartmouth-Hitchcock Medical Center in Lebanon, NH seeks an experienced Advanced Practice Provider to join its Endocrinology team. This full-time role (40 hours/week) covers inpatient diabetes care and outpatient endocrine management with a collaborative, patient-centered approach.

Applicants should be licensed APRN or PA in New Hampshire with NCCPA certification or AANP/ANCC board credentials. Diabetes experience or CDCES/BC-ADM is a plus.

Qualifications

• Graduate APRN program completed.
• NCCPA certification or board certified by AANP or ANCC.
• Eligible for APRN or PA licensure in New Hampshire.

Responsibilities

• Provide inpatient diabetes management on the Inpatient Glucose Management Service (GMS).
• Conduct outpatient endocrine clinics for discharged inpatients and ongoing diabetes/endocrine care.
• Collaborate with physicians and health care professionals to evaluate, treat, and educate patients.
