Job summary

Horizon Health in Illinois is seeking an Endocrinologist to join our growing team. This employed position focuses primarily on outpatient care with a small number of inpatient consults and a patient-centered approach to delivering endocrine care.

Horizon Health has an opportunity for an Endocrinologist to join our growingorganization.

Hospital Information

We are a critical access hospital and group of rural health clinics, butmuch larger in scope and size than typical rural organizations. We havecutting-edge technology, new offerings for the communities we serve, strongspecialty support, and still have the small-town approach and values thatmake our organization so special.

The current practice has one experienced nurse practitioner and staff,a certified diabetes care and education specialist (CDCES), and has beenrecognized by the ADA for our diabetes education program since 1994.

This employed position primarily will be outpatient-focused with a smallnumber of inpatient consults. Qualified candidates should be board certifiedor board eligible in Endocrinology, have excellent communication skills,and a patient-centered approach to providing care.

• We are currently staffed with 19 primary care physicians and providers,with specialty support in behavioral health, women’s services, generalsurgery, orthopedics, spine, pain, dermatology, podiatry, wound care,and cardiology.
• Additionally, we have 12 visiting specialists that see patients in theclinic each month.
• The hospital has a 24/7 hospitalist service and convenient care for patients.
• We have a physician friendly EMR.
• In 2023, we completed expansion projects including adding an outpatientdialysis center, home health services, pre-anesthesia clinic, and cutting-edgeradiology updates.
