Job summary

Essen Health care is seeking a compassionate Endocrinology Nurse Practitioner to join our team in outpatient endocrine care across our New York locations. You will work with an interdisciplinary team to deliver comprehensive endocrine services, manage diabetes, thyroid and other disorders, and help reduce readmissions.

Ideal candidates have experience in outpatient endocrinology, are board-certified as NP, hold a valid NP license with DEA, and are proficient with EMR systems.

Qualifications

  • Minimum 6 months of outpatient experience.
  • Active and unrestricted NP license.
  • DEA license required.
  • CPR certification (BCLS/BLS).
  • Endocrinology experience preferred; training available.

Responsibilities

  • Provide on-site endocrinology consults including initial assessments for new admissions and complex endocrine conditions.
  • Order and interpret labs including A1C, thyroid panels, metabolic panels, bone density tests.
  • Collaborate with endocrinologists and nursing staff to develop treatment plans.
  • Educate patients and families on diabetes management and hormone disorders.
  • Ensure accurate documentation in EMRs and regulatory compliance.

Job Summary

We are seeking a compassionate and dedicated Endocrinology Nurse Practitioner (NP) to join our dynamic team. In this role, you’ll play a vital part in delivering comprehensive endocrine care to patients, working closely with an interdisciplinary team in outpatient/acute settings.

Responsibilities
  • Provide on-site endocrinology consults including
    • Initial assessments for new admissions and complex endocrine conditions (e.g., diabetes, thyroid disorders, osteoporosis, adrenal disorders)
    • Routine weekly follow-up visits and care adjustments based on findings
  • Conduct comprehensive evaluations including
    • Gather medical histories and perform endocrine-focused physical exams
    • Order and interpret labs including A1C, thyroid panels, metabolic panels, bone density tests, and other diagnostics
  • Collaborate with endocrinologists, interdisciplinary teams, nursing staff, and facility providers to develop treatment plans, manage complex metabolic conditions, and reduce hospital readmissions
  • Administer and manage medications including insulin and endocrine therapies, monitor treatment responses, and adjust regimens appropriately
  • Educate patients, residents, and families on diabetes management, nutrition, hormone disorders, and medication adherence
  • Provide emotional support and counseling to patients and caregivers
  • Ensure accurate documentation in compliance with OSHA and governmental regulations, including thorough documentation in EMRs and transition summaries to ensure continuity of care across settings
  • Maintain familiarity with CMS regulations and NCQA HEDIS guidelines
  • Participate in quality improvement programs aimed at improving chronic disease management in SNFs
Qualifications
  • Minimum of 6 months of experience in a outpatient preferred
  • Active and unrestricted NP license
  • DEA license required
  • CPR certification (BCLS/BLS)
  • Master’s degree in Nursing (NP applicants)
  • Graduation from an accredited NP ProgramPreferred: Experience in endocrinology (training available if not)
  • Preferred: Familiarity with EMR systems (e.g., Sigma, Visual, PCC – training provided)
  • Strong organizational, communication, and time management skills
