Job summary

Responsibilities
  • Provides comprehensive care to residential patients; provides medication monitoring, education, and prescriber collaboration; provides information, education and support for clients; promotes wellness, recovery and skill building to sustaining better health; prevention training in the areas of wellness, recovery, managing symptoms, making healthy life style choices, recognizing and responding to relapse, managing crisis and achieving and sustaining recovery.
  • Acts as an integral, cooperative member of the agency team of professionals and maintains outstanding internal and external customer service and appropriate documentation.
  • Assists service recipient in developing an individualized treatment plan that is person centered, family centered, and service centered; monitors progress and participates in treatment plan reviews; updates treatment plans as appropriate; submits plans for medical review if applicable.
  • Conducts intake for recipients entering services, including explaining services, gathering information, ensuring understanding of forms, obtaining signatures on contracts and consents, and submitting materials for processing.
  • Conducts assessments and writes assessment summaries as assigned.
  • Assists recipients with linking to internal and external resources as outlined in the treatment plan and in coordination with Integrated Healthcare Services.
  • Maintains contact with referral sources, providing information on recipient progress, behavior and needs (with appropriate releases) and maintaining ongoing consultation when appropriate.
  • Assists recipients with discharge plans and provides follow-up services as assigned.
  • Monitors self-medication protocols, dosage, side effects, usage and documents patient reactions.
  • Provides medication training to the client or client’s family/guardian, to monitor proper dosage and watch for side effects.
  • Obtains laboratory specimens and assists doctors with examinations and continuity of care arrangements with other providers; monitors urinalysis, breathalyzer and other testing as assigned.
  • Meets and maintains applicable certification and licensure requirements.
Knowledge, Skills & Abilities
  • Effectively communicate via written, verbal, in-person and virtual methods.
  • Knowledge of confidentiality guidelines.
  • Knowledge of applicable regulatory guidelines and codes, agency policies and procedures, and federal and state laws pertaining to position, including SUPR rules.
  • Knowledge of main effects, side effects and interaction effects of a broad range of psychotropic medications.
  • Knowledge of both psychiatric and physiological bases for illness.
Qualifications
  • Education: HS Diploma or GED.
  • Experience: Staff utilizing electronic health records and/or other automated documentation systems; computer and keyboarding experience or ability to develop within required timeframes.
  • Certification/Licensure: Certification as Emergency Medical Technician; must have and maintain driver’s license and good driving record.
  • Background: All EMT position employees must successfully complete required background checks and pre-employment screening.
