Qualifications

• High School Diploma, GED or equivalent is required.
• Valid Driver’s License and acceptable driving record required.

Responsibilities

• Assist in developing individualized service and support plans and implement them.
• Provide support with daily living activities, skill development, and personal goal attainment.
• Assist individuals with transportation and community resources as needed.
• Implement activities that promote community inclusion, relationships and personal growth.
• Maintain a safe, clean, and supportive environment; ensure health and well-being of individuals served.
• Complete and maintain required documentation accurately and timely.
