Job summary

The Texas Health and Human Services Commission seeks a Transition Coordinator I to support individuals moving from facility to community living options. This role guides the transition process, coordinates educational opportunities, and works with an interdisciplinary team to identify supports.

You will travel up to 75% to meet with families, agencies, and providers. A Bachelor’s degree in a human services field and a valid Texas driver’s license are required, along with ability to collaborate

Qualifications

Bachelor’s degree in human services or related field with one year of related experience.
Valid Texas driver’s license required.
Background check and pre-employment drug screen are required.
Ability to work with individuals with intellectual disabilities and families.
Travel up to 75% is required.

Responsibilities

Attend regular work schedule and comply with shift requirements.
Provide consultative services to the IDT on placement planning and barriers to transition.
Develop tools to educate individuals and families about living options and transition process.
Schedule and conduct trainings on community living options and transition planning.
Coordinate and support two Community Provider Fairs annually.
Maintain records of transition activities and education opportunities.
