Responsibilities

• Evaluate patient's functional status and physical therapy needs.
• Assist physician in developing the therapy plan of care.
• Provide services within the defined scope of practice.
• Instruct healthcare team, patients, and caregivers regarding the plan of care.
• Complete all patient evaluations and develop the PT plan of care.

Skills

Current Physical Therapy licensure in state of practice

Current CPR certification

Ability to function in any home situation regardless of client conditions
