Job summary

Reliant Management is seeking an LVN to provide nursing care under the direction of an RN in California. The LVN will assist with assessments, care planning, direct patient care, and supervision of unlicensed staff.

The role requires California LVN licensure, ability to read and follow physician orders, and a focus on patient outcomes and family satisfaction. Prior rehab/geriatric experience is preferred, with strong teamwork and PHI safeguards.

Qualifications

• Graduate of an approved school for practical or vocational nursing is required.
• Must be currently licensed by the State Board of Nursing.
• Experience preferred in rehabilitative or geriatric nursing.
• Must read, write, speak and understand English to ensure safety of patients and visitors.
• Must provide verification of TST (tuberculin skin test) as required by state law.

Responsibilities

• Evaluates patients via data collection and observation and reports changes to RN.
• Contributes to individualized patient goals and plan of care.
• Delivers direct patient care and administers medications per orders.
• Documents accurately and thoroughly and communicates data to RN/physician.
• Assists with discharge planning and coordination of care with the team.
• Monitors unlicensed staff performance and ensures policy adherence.
• Maintains confidentiality and PHI protection.
