CenterWell Home Health is seeking a dedicated Home Health Registered Nurse to provide one-on-one skilled nursing care in patients’ homes. You will work with a clinical team to create and execute individualized care plans, monitor patient progress, and educate families on disease management and treatment options.

Responsibilities include wound care, medication administration, assessments, and coordination with physicians and the Clinical Manager.

Competenze

• RN with current and unrestricted license (US).
• Experience in med-surg, ICU, ER, or acute care settings.
• CPR certification and ability to work in patients' homes.

Mansioni

• Provide skilled nursing care in patients' homes, including meds administration and wound care.
• Develop and implement individualized care plans with physicians and clinicians.
• Monitor patient conditions and report changes to the physician and Clinical Manager.
• Educate patients and families on disease management and treatments.
• Maintain accurate care records and coordinate with other professionals.
• Coordinate with the Clinical Manager on patient progress and documentation.
• Oversee Home Health Aides and ensure proper documentation completion.
