Job summary

Virtua Health is seeking a Registered Nurse (RN) for Home Health Care (Full Time) in South Jersey. The role focuses on comprehensive patient assessments, care coordination, and education in patients' homes.

Responsibilities include supervising Home Health Aide, implementing treatment plans, and ensuring compliance with documentation standards and safety practices. NJ RN license and hospital or community health experience are required.

Qualifications

• Must have a valid NJ RN License.
• 1–2 years recent work experience in hospital settings (Med/Surg, ER, OB/GYN, Peds, MICU, CCU) or community health.
• Broad knowledge of nursing practice and ability to apply in home health.
• Bachelor’s degree preferred.

Responsibilities

• Assess, diagnose, plan, implement, and evaluate patient care in patients' homes.
• Coordinate care plans with physicians and healthcare team.
• Supervise Home Health Aide in the home.
• Educate patient and family on disease process, medications, safety, and wellness.
• Maintain documentation standards and safety practices.
