Job summary

River Valley Home Health and Hospice is seeking a licensed physical therapist to deliver comprehensive physical therapy services in home health visits, coordinating with patients, families, and caregivers to support physician plans of care.

The role emphasizes interdisciplinary assessment, patient education, and discharge planning to achieve high-quality outcomes and patient satisfaction in the community setting.

Qualifications

• Licensed to practice as a physical therapist in the state.
• Graduated from a CAPTE-accredited PT program.
• Two years of relevant PT experience preferred.
• Good verbal and written communication and organizational skills.
• CPR certification current.
• Must be a licensed driver with insured vehicle.

Responsibilities

• Assumes responsibility for ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative (if any), and caregiver(s).
• Provides patient, caregiver, and family counseling and education.
• Prepares clinical notes and progress summaries based on attainment of goals.
• Provides PT services to patients according to a physician's plan of care.
• Assesses muscle strength, mobility, gait, ROM; evaluates rehab potential.
• Directs physical therapy treatment and educates patients and families/caregivers.
• Identifies needs for other home health services and refers as necessary.
• Participates in discharge planning and peer consultation.
